Completing a CACP/SMMA Follow-Up
For Alberta pharmacies only — learn how to schedule and complete a follow-up from an existing care plan in MedEssist.
1. Select "Care Plans"
2. Locate your patient's Care Plan and click "Open"
3. Click "Follow Ups"
4. Click "Schedule Follow Up"
5. Click "Register"
6. Select the date you would like to have the follow-up appointment. Once booked, select "Close"
7. From your Schedule tab, click the "Tasks & Follow Ups" icon to view your list of upcoming/due follow up appointments
8. Select the Documentation icon beside your patient's name
9. Click "Follow Ups"
10. Choose the Care Plan for the Follow-Up Assessment
11. Enter the date of the Follow Up
12. In Progress Notes, a drop-down menu will appear, allowing you to select the appropriate statement for each patient's Drug Therapy Problem status. You can select multiple statements if there are multiple Drug Therapy Problems
Tip! You also have the option to type out your own custom notes.
13. Once your Progress Notes are complete, click "Save" to update the patient's Care Plan
Tip! Schedule your patient's next follow up appointment right away by pressing the "Schedule Follow Up" icon and following Steps 4-8
