Creating a New Smart Form

Create a completely custom HIPAA-compliant online form that can be texted/emailed to patients, booked on your website, and more!

  1. Click "Smart Forms" on the dashboard sidebar.
  2. Under "Create New Form", click "Start"
  3. Build your questions below.
  4. Once you are done, click "Save" at the bottom of the form.

If you're just using this form for internal pharmacy use or will be sending the link directly to patients each time, you can stop here.
