# Create a Smart Form

## Create a completely custom HIPAA-compliant online form that can be texted/emailed to patients, booked on your website, and more!

1. Click "**Smart Forms"** on the dashboard sidebar.

2. Under **"Create New Form"**, click "**Start**"

3. Build your questions below.
4. Once you are done, click "**Save**" at the bottom of the form.

If you're just using this form for internal pharmacy use or will be sending the link directly to patients each time, you can stop here.
