Edit Smart Forms Update and change form questions, conditional logic, treatment options, and more. Click "Smart Forms" on the dashboard sidebar. Under "Open an Existing Form", click "Continue" Select the desired smart form to edit Once finished, click "Update" at the bottom of the edited form to save your changes
